Membership Plan

This plan is designed to make quality dental care more affordable and accessible for our patients. It is not dental insurance. Membership provides discounts and preventive services at Cochituate Smile Center only.

Plans to choose from

Choosing one of the plans includes the following services during a 12-month term:

Adult Wellness Plan: $399/yearly or $35/monthly

• 2 Professional dental cleanings & exams
• Annual digital X-rays (as needed)
• Oral cancer screenings
• Emergency exam (1 per year, if needed)
• 15% discount on additional dental treatment (excluding orthodontics, products, and third-party services)

Child Wellness Plan: $299/yearly or $26/monthly

• 13 and under only
• 2 Professional dental cleanings & exams
• Annual digital X-rays (as needed)
• Fluoride treatments
• 15% discount on additional dental treatment (excluding orthodontics, products, and third-party services)

Perio Care Plan $599/yearly or $35/monthly

• 3-4 Periodontal maintenance cleanings
• 2 Doctor exams
• Annual digital X-rays (as needed)
• Oral cancer screenings
• 5% discount on periodontal and additional dental treatment (excluding orthodontics, products, and third-party services)

Services cannot be transferred to another person, carried over to the next year, or refunded if not used. Membership fees are due in full at the time of signing the contract if choosing the yearly payment. If you choose the monthly payment, there is an initial fee of $199 the day of signing and the remaining balance will be split into $35/6 months.

Rules of the contract

Term & Renewal

• Membership is valid for 12 months from the date of enrollment.
• Renewal is required to continue benefits.
• Membership will not automatically renew; patients must re-enroll each year.

Limitations & Exclusions

• This plan is not insurance and cannot be combined with dental insurance, discount plans, or third-party financing discounts.
• Membership benefits are valid only at Cochituate Smile Center.
• Membership does not cover: referrals to specialists, hospital charges, lab fees, or products.
• Missed appointments may result in forfeited services.

Payment & Responsibility

• All payments are due at the time of service.
• Membership fees are non-refundable.
• Exceptions may only be made at the discretion of Cochituate Smile Center in special circumstances.
• Member is responsible for scheduling and keeping appointments to use benefits.
• For patients who choose monthly payments, we will require a card to keep on file for our automatic system to charge every month, you are responsible for ensuring that the registered card has available funds.
• If for any reason the payment is not processed, we will contact you in the first month; however, more than 2 consecutive missed payments may result in contract cancellation.
• To continue using the plan, you must fully pay all accumulated charges.
